From 911 call to survivor celebration: How Dane County uses data to inform and strengthen our system’s high-quality resuscitation efforts

Summary: We've created a process from 911 call to the year-end survivor celebration, allowing us to support timely and actionable feedback along with system-wide analysis. These efforts have led to our highest number of neurologically intact survivors in 2022.

Synopsis: It has long been the goal of Dane County EMS to be a leader in sudden cardiac arrest (SCA) survival. We provide support to 23 EMS agencies spanning career, volunteer, urban, rural, ALS, and BLS across 1,200 square miles. Our first submission to the CARES registry was in 2016, with just under 2,400 submissions by the end of 2022. We held our first local Resuscitation Academy in March of 2020, and two months later implemented a process to provide crews actionable and timely feedback on every prehospital SCA resuscitation. 865 cardiac arrest summaries were distributed by the end of 2022, with significant improvements in chest compression fraction, reducing prolonged interruptions in compressions, post-ROSC EKG capture, and compression rate stability.

Additionally, we’ve seen improved data submission compliance, greater context and detail in prehospital report narratives, and strengthened partnerships with our 911 center and local healthcare systems.Concurrently, our 911 center has worked to improve cardiac arrest recognition, bystander CPR rates, and reduced first compression times. This framework ensures the 911 audio for every prehospital resuscitation is reviewed, allowing targeted feedback and recognition for our 911 call takers.These combined efforts led to our highest number of neurologically intact survivors in 2022, showing how measurable data, with actionable improvement goals, can lead to significant advances in prehospital resuscitation from 911 pickup to survivor celebration. This presentation will provide attendees with a multi-disciplinary SCA review and improvement framework through which we have found consistency and success, along with the lessons we’ve learned along the way.

Learning Objectives:

To provide perspective and lessons learned on implementing a consistent and timely prehospital cardiac arrest performance review process from 911 call pickup through success recognition.
To detail how our EMS office and 911 center have partnered, demonstrating how two separate processes can work towards a common goal and reduce duplicate efforts.
This presentation will include components of a high quality collaborative cardiovascular and resuscitation program in a dispatch, EMS and / or healthcare system that has demonstrated improvement in survival from SCA.
This presentation will include strategies for implementing data collection and quality improvement programs in an ECC system to support accountability and reporting.
This presentation will describe one or more survivor / rescuer stories that can be used to illustrate an effective chain of survival for cardiac arrest.
