Summary: The VA has incorporated elements of disruptive inter-professional team dynamics into a mock code simulation in order to provide practice in eliminating the negative impact poor team dynamics has on quality ACLS delivery.

Synopsis: Disruptive behaviors among physicians, nurses, and other healthcare workers have led to adverse events, errors, or compromises in quality of care according to a VHA survey. In a survey of VHA Redi Mock Codes it was found that participants did not meet expectations in categories of team dynamics such as designating responsibilities, closed loop communications, and clear messaging. To minimize impact of disruptive behavior in a high stakes patient care setting (ACLS), a simulation was created to provide practice and reinforcement of de-escalation and redirection while maintaining focus on the care of the patient.

Learning Objectives:

List and describe complicated team dynamics and PMDB characteristics.
Implement the appropriate ACLS/BLS protocols with disruptive staff present in the scenario.
This presentation will include components of a high quality collaborative cardiovascular and resuscitation program in a dispatch, EMS and / or healthcare system that has demonstrated improvement in survival from SCA.
This presentation will include strategies for implementing data collection and quality improvement programs in an ECC system to support accountability and reporting.
